Looks like the editors didn’t include my SSRS report code in the "Download the Code" archive above.
If you send me an e-mail, I will mail you a ZIP file that contains the report code.

I just completed a very similar project.
We have a set of scripts that act as the definition (These databases get built much like c# files get built).
Our innovation was to require comments in the CREATE TABLE scripts. These immediately precede any column or table names and start with "---".

The same way xml documentation in .net code requires comments that start with "///"
